Each appointing power shall report immediately to the director of personnel, in such
detail and on such forms as the director of personnel may prescribe: Every appointment,
suspension, separation, reinstatement, restoration, layoff, transfer, reassignment,
absence, the refusal or failure to accept an appointment on the part of an eligible
certified for appointment, and changes of duties or of compensation.
